Clinic

Parents must complete the Health Services Consent online in Skyward Parent in the parent portal, if you would like to opt out of any health care services.

You will find a link to the Parent Portal on our home page, and instructions on how to create an account and use the portal, under Student Services.

It is very important that every student have updated Emergency Student Contacts on file with the school. Contact lists can be updated on our website, under Parents.  Make sure you are also removing outdated contact information.

Parental consent does not apply to emergency services. In the event of an EMERGENCY, I understand that the school will access the 911 emergency medical system

Illness

Please keep your student home if they have a temperature above 100.4, until they are fever-free for 24 hours without the use of fever-reducing medications

Injuries

Please provide the school nurse with medical documentation of any injury that requires restrictions/accommodations ordered by the physician. Medical documentation is required to be excused from PE.

Students interested in playing on a school-sponsored sports team (soccer, volleyball, basketball, or track) must have an up-to-date sports physical on file before tryouts. Sports physicals are valid for one year.

All students must be up-to-date with their immunizations. Students entering 7^th grade may need a Tdap booster, which must be recorded on an updated DH680 form from the physician. Please talk to your health care provider about whether your child is up-to-date with immunizations. The immunization form should be submitted to the main office for processing.

If your student had a certificate with a Code 2 and an expiration date, the certificate expires after that date.  A new certificate is required, as the student is missing important vaccine updates.

Religious exemptions are only available from the Florida Department of Health.

The school health assistant can administer prescription and over-the-counter medications during the school day, if needed. The parent/guardian must bring the medication to the clinic in the original pharmacy bottle with the label attached. If you would like to keep over-the-counter medication in the clinic (e.g., Benadryl, eye drops, TUMS, etc.), the parent must bring in an unopened bottle. Please complete the medication packet from below and bring it to the clinic along with the medication. The clinic does not stock medications to administer to students. Students may not bring medication to school or carry it in their backpacks, except for Tylenol/Advil, for their personal use

**Please note: In accordance with OCPS policy, if students are found with medication or unauthorized inhalers, EpiPens, etc., the items will be taken and the parent/guardian will need to come to the school to pick up the items.

Students with Asthma and Inhalers

If your child has asthma, you can store an inhaler in the clinic. If you would like your child to self-carry an inhaler in their backpack, you must complete the Self-Carry form and have it signed by your family’s health care provider. Students are not permitted to self-carry an inhaler without a completed form. Please obtain a current asthma action plan from your provider.

Students with Severe Allergies and EpiPens

If your child has severe allergies and requires an epinephrine injector (e.g., EpiPen, Auvi-Q, etc.), you can store it in the clinic. Students may also self-carry an injector in their backpack as long as the appropriate paperwork is completed. Several forms must be completed by the parent/guardian and the health care provider. Please make an appointment with your health care provider to have the forms completed and signed as soon as possible.

Students with Pancreatic Enzymes

If your child takes pancratic enzymes with meals, you can store them in the clinic, or the student can self carry them, provided the self carry form below has been completed and is on file with the clinic.

Students with Diabetes and Diabetes Testing Supplies

If your child is diabetic and needs to self-carry testing supplies and insulin, several forms are required from the health care provider. Please make an appointment with the health care provider to obtain a current copy of the Diabetes Medical Management Plan and for the appropriate signatures on the Self-Carry form.
